Tongue and pulse diagnosis: how TCM intake works in a licensed clinic

How licensed TCM diagnosis works: tongue color and coat, pulse positions, pattern differentiation. What to expect at an NC acupuncture intake. Indian Trail.

People are surprised that needles wait. In a Traditional Chinese Medicine clinic the first work is looking, asking, and touching: tongue (wang) and pulse (qie), plus a history of sleep, digestion, temperature preference, and mood. Licensed acupuncturists in North Carolina practice under N.C.G.S. § 90-452. That statute authorizes acupuncture and related techniques — it does not authorize diagnosing Western diseases as a physician would. Pattern differentiation sits beside your MRI, not on top of it.

Why this is not a Western disease label

Two patients with “low back pain” can have opposite TCM patterns — Cold-Damp obstruction versus Kidney deficiency. Point selection follows the pattern. That is why a cookbook “back-pain protocol” is not how this clinic works. It is also why ChatGPT-style symptom lists are a poor substitute for a live tongue and pulse.

Tongue: five things we actually look at

Body color (pale, red, purple), body shape (thin, swollen, scalloped), coating color, coating thickness and moisture, and spirit (shen) of the tongue. Coffee and colored drinks right before a visit stain the coat — skip them if you can. A purple body with distended sublingual veins is a different conversation than a pale scalloped tongue. Classical pulse study (including texts such as the Bin Hu Mai Xue) still informs training; modern research on inter-rater pulse reliability is mixed, which is why we never treat pulse as a standalone lab test.

Pulse: more than “fast or slow”

Three positions on each wrist, superficial and deep, looking for qualities such as wiry, thready, slippery, or choppy. It is a trained tactile skill, not a fitness-watch heart rate. Blood-pressure medication, coffee, and rushing from the parking lot all change what we feel — say so.

What we do with the pattern

Needles, and when indicated herbs, cupping, or gua sha, follow the pattern. If the pattern is deficiency without stagnation, we may skip scraping even if you asked for gua sha. That refusal is diagnosis working. Bring your medication list; herbal recommendations are conservative during pregnancy, active oncology, and polypharmacy.

What is tongue and pulse diagnosis in acupuncture?

It is the TCM exam: inspecting the tongue and palpating radial pulses to identify a pattern of disharmony, then choosing points and adjuncts. It does not replace imaging or physician diagnosis.

Should I brush my tongue before the visit?

Ordinary hygiene is fine. Avoid scraping the coat off minutes before intake and skip staining drinks if you want an accurate coat reading.

Is pulse diagnosis scientifically proven?

It is a traditional skilled exam. Studies of inter-rater reliability are mixed. We use it with history and tongue, not as a standalone proof.
